According to Reaney & WIlson's A Dictionary of English Surnames, this indicates that one's ancestor came from a place called Tilney in the county of Norfolk, England.

And according to A Dictionary of English Place-Names by A. D. Mills, the place name either meant "useful island" or "island of a man called Tila".
