I don't go by my middle name, but my husband did. This wasn't a choice he made later in life, though, it was choice made by his parents at his birth in order to avoid confusion with his father, after whom he was named.
I know a couple who named their son Steven Alexander and call him Alex. His first name is Steven because that's his father's name. I don't see the purpose of using the father's name as a first name if you're going to give him a different middle name and he won't be a Junior. I can see giving the father's name as a middle name. So in this case, I would have named him Alexander Steven.
