If you had twin girls and their MNs had to be connected what would you call them? What would you call their siblings?

I would call their siblings what I would want to name anyone anyway. ;-) But if I did male siblings/twins with thematic middle names they would be:

Boris Koloman
Or Robert Koloman if I decided to give my offspring something less challenging. The first theme is pretty straightforward but if the second doesn't jump out:

Boris is the reason that my mother has suggested I perhaps will not be relied upon for grandchildren. :-P

Stella Tavaita & Ruby Cecilia - Tavaita is one of Nate's aunts and Cecilia would be after one of my aunts.

Mary Elanor and Rosamund Estel.
Elanor means "star sun" in Sindarin (one Lord of the Rings dialect of Elvish) and Estel means hope in Sindarin.

Valentine Tara and Virginia Elise (Valentine Tara is my mother's full name, Virginia Elise is dh's mother's full name)
Marigold and Rosemary-Both of our favorite flowery names
Verity and Blythe-two of my favorite virtue names (also Clementine)
Opal and Ruby-my two favorite gemstones (also includes Pearl)
Louisa and Frances-first names of two of my favorite childhood books, Louisa Alcott and Frances Hodgson Burnett
Nola and Louisiana-Dh was born in Louisiana, in New Orleans (Nola stands for New Orleans, Louisiana)
For boys, I'd probably connect their middle names by honoring grandfathers, honoring loved ones who've passed, honoring the Saint's football team, honoring two of dh's favorite comic book heroes,nature names (like River and Jasper), or some kind of mythology.

Dorothy (or Martha) Ann and Hermione Alexandra.
Their siblings would have normal names (to me, at least). Severus Alexander, Frederick George, Edward Francis, Horatio Nelson, Charlotte Sophia, Eleanor Frances, etc.
Actually speaking of the siblings, it would be funny if there were instead triplets and Dorothy (Dottie) and Hermione had triplet brother Severus Alexander, because of all the As. And then twins Edward Francis and Eleanor Frances.

As for siblings, I'd just go with my usual names I'd name children (Alexandra, Elizabeth, William, Charles, James, etc.). If I were to carry on the theme with girls' middle names I might also use Susan (as in Black-eyed Susan), Bluebell, Daisy, or Glory (as in Morning Glory). They're all flowers that have personal significance to me.
