The funniest thing is that my brother still calls me by my middle name, rather than my first. (I went back to my first because I have a sister-in-law named Elizabeth.) The nickname thing is harder, I think - unless you really, really insist on it.
But the thing is: You have to insist on it, and some people aren't able to. I babysat for a woman who converted to Judaism and changed her name to Ruth - no one ever called her it. She was just Joy.
