I can't (and don't want to) picture it on a girl, probably because I'm so attached to Mikael and Mihail and Mihael and Michael for boys. I'm also pretty sure it would be often confused with Michael, unless Michal-on-a-girl is common/usual where you live. It might be best just to use it as middle name to avoid confusion. Besides, Kassandra Michal is quite pretty. :)
Michal is a very popular girls' Hebrew name in the Jewish community. I've known several online and a couple IRL. It seems to be popular for girls bat mitzvah age to three years older. To be honest, I've never seen it used on a non-Jewish person, but doesn't mean they're not out there. :) I rather like it. In Hebrew, it's pronounced "mee-KHAHL."
I don't like the look of Kassandra and the way I'm pronouncing Michal, the two names don't flow very well together. This combo isn't for me.
As for nicknames, outside of Micki/Mickie and Mimi, the only nickname I'm aware of (that I've heard used) is Michale.

I realize Michal has meaning and history as a girl's name. But sometimes it's not worth the headache to use it if there is confusion. "No, not Michael as in the boy's name. M-i-c-h-a-l. It's a Hebrew girl's name." How old would that get?
As a combination, I don't like how close the "k" sounds are in Michal Kassandra.

I so agree.
This doesn't initally strike me as feminine, but I see there is a female Biblical Michal. ("MEE-kull" or "MIE-kull"?) As to the middle name, I prefer the Cassandra spelling and like the name. However the "k" sound in Michal and the "k" sound in Kassandra are so close to one another, that I'm not enjoying hearing it repeated. Kassandra Michal spaces those "k" sounds out and is much more pleasing to my ear.
With Michal as a first name, and assuming the "MEE" pronunciation, Mimi, Kiki or Callie could work for nicknames, I think.

I've actually only known it on girls. In Israel, I think it's fairly common (girls only, like in the bible). This is mi-KALL, right? I've seen Michael on a girl a couple of times here too, also in spellings like Mykel and Mykal.
Micki is always cute for a girl, I think. You could probably also get away with Millie, which I love as well.
Kassandra is not really my thing, and with Michal, I hear too many K sounds, but that's just my personal bias.
