OK. So Ruth stays as the mn, and you're looking for a 3 syllable fn that also works with siblings Isaiah and Naomi. Got it.

Mariam / Maryam Ruth (Mara?)
Hadassah Ruth (Haddie, Dassah?)
Tabitha Ruth (Tabby, Tibby?)
Tamara Ruth (Mara?)
Samara Ruth (Sam, Mara?)
Magdalene Ruth (Maggie?)
Margalit Ruth (Margie?)
Daniella Ruth (Ellie?)
Aviva Ruth (Avi, V, Viv, Evie?)
Rebecca Ruth (Rebi, Becky, Bex?)
Gabrielle Ruth (G, Gabby, Bree, Ellie, Belle / Bella?)
Susannah Ruth (Susie, Annie, Zan, Nan?)
Shoshannah Ruth (Shannie?)
Or, you could use Ruth as the fn, with nn Rue / Ru. That might throw off your pattern though.
