Honestly, getting pregnant was the worst idea Demi ever had. She vomits so much during the first trimester that she becomes dangerously dehydrated, spends the second trimester just trying to recover from that, and then suffers through numerous early labour scares and almost comically swollen feet (all the way up to her knees!) from virtually the very beginning of the third. She can barely get out of bed for the majority of nine months and has to let her advisers and Consort essentially rule in her stead. But in the end (after many hours of grueling labour) she finally has an heir the people would accept without complaint: a child of her own body.
(It really is no business of anyone's beyond her own and Gem's who the biological father is, though some "insiders" strongly suspect a young alchemist named Leofric Archibald, a childhood friend of Gem and former classmate of both Gem and Demi at university who happened to be visiting Lai Shi around the right time.)
The new Crown Princess has to wait a while for a name, because while Gem holds their new little daughter to Demi's chest and tries to be supportive and encouraging and not let on how terrified she is, the court physician (Weaver) fights to keep Demi herself from dying due to a massive hemorrhage. But finally, once the immediate danger has passed, Demi and Gem settle on Iris Thaleia Seren.
But little Leia is heavily jaundiced and just wants to sleep, and it becomes a constant battle to wake her at all to feed. Demi develops an awful case of mastitis, but refuses the sage potion offered by Weaver, insisting that she needs to feed her baby - who, after all, is steadily losing weight. They could, of course, employ a wet nurse easily enough if need be, but the problem of a baby who needs milk but won't take it and a mother who has too much hardly seems logically solved by a wet nurse. With a combination of sheer stubbornness (and a dose of desperation) and the invaluable support of both Gem and Weaver, Demi and Leia finally work things out, and, after a very rocky first three and a half weeks, things are looking up. Leia is finally gaining (though not rapidly) and is not nearly so prone to sleeping all day now that her jaundice has cleared.
(This rocky start has not quite escaped the notice of the populace despite official assurances that neither the Empress nor the new Crown Princess are on the brink of death.)
Gem has offered to carry the next child (and any additional children they might have) herself. In fact, she's quite resistant to the idea of Demi ever getting pregnant again for fear of a repeat of all of that!
