Will you be changing the child's name? No, but we might add a second middlename.
Will you allow the parents to keep in contact with the child after you adopt? Likely not, we want a child from abusive backround or a child whose parents are drug addicts.
Will you accept teen mothers or fathers? no
Will you accept children with a physical disability? If so, mild, moderate, or severe? yes, severe or moderate, but not terminal illness
Will you accept children with an emotional disability? If so, mild, moderate, or severe? yes, severe or moderate

Is there anything else you want to add or say? We want a child with special needs (for example mental retardation, autism, alcohol- or drug-related birth defects, severe chronic illness, severe psychiatric problems, blindness etc.). Thank You!
