I searched archives for the very first post of Count Along CAF. It was posted by Eden, and here are her directions (same for this game):

"I played this on another board and it was a lot of fun. Go by the directions for each name and count along in the name banks to get the name. Start at the beginning for the first name and after that, start counting with the last name you used.

For instance if the bank were:

Girls: Scarlett, Andrea, Omega, Jemma, Clio, Joelle, Deniece, Martha, June.

And the instructions said:

DW: FN - Count 3 MN - Count 4, the name would be Omega Deniece. Then when you did the first dd, you'd start at Deniece and count.

Do the same for the boys.

---

If you get through the bank, just start over at the beginning, skipping any name you've used."

The DH's FN on this game says 'favorite pizza topping'. Let's say you like cheese pizze only. The word 'cheese' has 6 letters, so you go to the name bank for guys and find the 6th name. In this case, it's Emmanuel. You do the same thing for every name, counting the number of letters in each word of your answer, then finding the corresponding name in the name banks.

I'm not sure if you're supposed to, but I omitted a name once I used it so that I didn't end up with two family members with the same exact name.
