Polish, belarusian or, less likely, German surname... Anyone knows its etymology?

The name Czołgosz is definitely Polish. It's listed in Hoffman's dictionary of Polish surnames but no satisfactory origin is given for it.

but it shouldn't have such a complicated etymology... Doesn't it? I mean... Czol must be one lexeme and "gosz" must be the other lexeme... or "Czolg" and "osz"...

Polish "czołgać się" means "to crawl". There may be a connection.
