wouldn't really bother me, as long as they're both names you really like, and would use them for non-twin sibs too. If you choose two names with the same letter just cos you want them to sound 'twinny,' then I don't really like the idea.
But I would give them different middle initials. It's much less confusing (especially at school, when you sometimes have to put your initials on the back of your work rather than your whole name) to have twins whose initials are AJN and AMN than AJN and AJN.

It depends on the middle initial. I have a friend with twin brothers, and they had matching first and middle initials. That, along with having the same birth date and place, apparently created a lot of confusion for them with some government offices.
I don't mind twins having just the same first initial, though. Many non-twin siblings do, so I don't think its a big deal.

Yes, I would. I am completely against same initials names for twins. But, I would only change it to a name I liked; so, I wouldn't choose Eva because it's close to Ava, if I really didn't like it. I would go in a new direction completely.

If you love the name enough then no. If it's not like 'they must have matching first initials ' then I would go with the names i loved and wanted to use.

As long as the names aren't too similar (and Ava and Adrian are not too similar) I wouldn't change the letter simply to not have them begin with the same letter.
