First of all, thank you for a great game Rachel!
Well, I think you know how to play... 'count a long' to the proper place in the namebank, use the name, then delete it. If this doesn't explain it (which it probably doesn't) you may want to check out Rachel's count-a-long CAF. She did an excellent job.
The only difference is you can choose before you play the game/look at the namebanks if you want to count from the beginning of the nambank or the end.

DW: FN: Favorite Fruit MN: Favorite Wild Animal
DD: FN: Mice or Spiders? MN: Your last name
DD/DS: FN: Your favorite candy MN: Your favorite book/ FN: Favorite Fast-food place/ Least Favorite Vegetable
DD: Your middle name (if you have none, count 15) MN: An instrument you play (if you don't play one count 2)
DS: FN: The current ruler of your country MN: Favorite music artist
DS: FN: Soda-pop or Water? MN: Favorite type of flower
DD/DD: FN: Sugar, Spice or Everything Nice? MN: Jane or Pandora?/ FN: The number 1000 name the year you were born (female) MN: The number 1000 name the year you were born (Male)
DS: FN: Favorite Color MN: Video Tapes or DVDs?
DD: FN: Are your toe nails painted? (yes/no) MN: What color are they painted? (If none just say "natural")
