Most people I know use some form of their initials or their surnames as a nickname. I'm "HC Beez" (see the post about Church), and one of my friends has the last name Caplinger, nickname Cappy.

A lot of people I know shorten their surnames and go by that eg a Sarah Madison might go by Maddie, or Adam Smith by Smithie.
