Everyone took the optional survey. Here's the spreadsheet from that:

In order to actually form pairs, I ended up writing out people's info onto cards and physically swapping them around until I was happy with them. (The reason other people's usernames are written on some cards is that I don't pair people up who have been paired in a previous iteration of the game). I kind of took a holistic approach based on the information I had in front of me to choose.
