I think it would be better as a middle name, simply because I find Mac- surnames a bit irritating, no matter the reason behind them. MacTavish strikes me as slightly less annoying than MacKenzie, but I'd go with Eli MacTavish Thompson M. Sounds really classy to me--and if he's already going to have two surnames, he really deserves a first name that sounds like a fn, IMO.
