well...it sounds to me an Italian surname.
Mind you, I'm Italian and in the end Portugal, Spain, France etc share a common root in language which is latin. So the surname could easily be spanish or portuguese of italian.....

The surname Formosa is Italian, it exists especially in Southern Italy: CAMPANIA (the region of Naples), CALABRIA and SICILIA.
This surname means "beautiful", "well shaped". Both in Italy and Spain there is also the surname Formoso.
